An Advance Care Plan can empower and prepare individuals, their loved ones, carers and healthcare professionals for important healthcare decisions in the future. This reduces anxiety and improves outcomes for all involved.
ACPA works to ensure advance care planning discussions become a key part of routine health care conversations across Australia and delivers an extensive range of programs, services, training and education to foster innovation and research in this important area.
