Description
My health for life is a free, healthy lifestyle initiative proudly funded by the Queensland Government through Health and Wellbeing Queensland.
The initiative is led by Diabetes Australia in collaboration with Key Partners – Ethnic Communities Council of Queensland (ECCQ), Stroke Foundation, and Queensland Aboriginal and Islander Health Council (QAIHC).
My health for life connects adult Queenslanders 18 years and over with qualified Health Coaches, online support and more to help them make healthier choices to reduce their risk of developing chronic conditions such as type 2 diabetes, heart disease and stroke.
Through six personalised coaching sessions over 14-18 weeks, participants discover practical ways to improve their daily habits around eating, moving, and wellbeing.
It’s provided in a variety of ways, from groups to phone or online coaching - locally, at home or at work - with culturally adapted versions.
