Country to Coast Queensland (CCQ) is an independent, not-for-profit organisation delivering the Australian Government's Primary Health Network (PHN) program across Central Queensland, Wide Bay and Sunshine Coast. We work with communities, clinicians and partners to strengthen primary care, improve access to services and support people across our region to live their healthiest lives.
Our vision is for everyone in our region to live well. Our mission is to work with community to deliver leading primary health care through collaboration, evidence and local insight.
